Looks to me that the batter is ready for the pitch. There isn't a requirement to step back from the windup position, is there?

I dont see QP - 8.01a ( may take a step back) - not required.

I had to watch the video twice because I thought the PU may have got caught off guard but everyone is set so I don't know what the problem was.

This was not a quick return pitch. The batter was ready and the pitcher came to set and the batter was not "off guard".

Even though the step back isn't required ...if he does it in all his pitches, and then doesn't ...could that be the issue?

Only if the umpire was surprised by the move.

Definitely a bad call. Everyone was set. I think the umpire possibly looked away or blinked, because there is no way that is a quick pitch.
